NDIS SERVICES FOR PLAN OR SELF MANAGED

Assessment and treatment services are available for  NDIS clients - both Plan or Self Managed. Services may include: Initial OT Functional Assessments, Home Assessments (Equipment Prescription), Driving Assessments, Pain Management and Wellness Coaching, and necessary follow-up.

NDIS services also include Social Worker and Physiotherapy services.

CASE MANAGEMENT FOR LTCS, CTP, DUST & DISEASE & WORKCARE

Our Occupational Therapists are approved to provide Case Management services under Lifetime Care, CTP Insurance and WorkCare. Our clients may have traumatic acquired brain injury and/or spinal injury from serious workplace injuries or motor vehicle accidents. We coordinate treatment and necessary services so the client achieves their goals, whilst supporting the client and families involved.

OCCUPATIONAL THERAPY: DRIVING ASSESSMENTS, HOME & ADL ASSESSMENTS

Our Occupational Therapists are registered in the specialty area of Driving Assessments. Clients may have a disability or may have early signs of Dementia or Alzheimers requiring assessments and review. Our services extend to bus and truck assessments.

In the strive for independence, our Occupational Therapists provide clients with Home Assessments, to assess their functional capacity and participation in activities of daily living (ADL). Outcomes determine potential equipment or training needs.

MEDIATION & CONFLICT RESOLUTION

In the event there is conflict, we offer trained Mediators to resolve outstanding agreements, tensions or miscommunication. This may be a workplace or personal matter.

WORKPLACE HEALTH & SAFETY

Our Occupational Therapists have specialised training in assisting businesses to manage risk via inspections, system implementation and training. Services also extend to include Workplace Assessments, Job Analysis and Ergonomic Reviews.

PAIN MANAGEMENT & WELLNESS COACHING

Our services include 1:1 Pain Management or Wellness Coaching utilising the results driven Abilita Program. Measurable outcomes can be determined following participation in modules to improve sleep, reduce flare-ups and assist with upgrading activity.

PHYSIOTHERAPY

Treatment areas include:

- CTP and workplace injuries;

- back and knee pain;

- sports injuries;

- arthritis;

- aches, sprains and injuries;

- incontinence;

- rehabilitation from brain injuries or assisting with treatment of neurological conditions, like Parkinson’s disease or multiple sclerosis;

- chronic diseases like diabetes, osteoarthritis, osteoporosis and obesity;

- managing after a stroke;

- recovery from broken bones;

- rehabilitation after surgery; and

- Occupational Health.

SOCIAL WORK SERVICES

Our services include:

- Initial Social Worker Assessments and Recommendations;

- Assistance with NDIS applications alongside support for planning meetings, ongoing liaison with NDIS Support Coordinators and assistance with Plan Reviews;

- NSW Civil and Administrative Tribunal (NCAT) Guardianship and Financial Management applications and hearings;

- Centrelink applications, alongside ongoing support until the application process is finalised;

- Housing NSW - Public Housing applications, Private Rental assistance, Private Rental Subsidy Scheme (PRS) applications, homelessness;

- Temporary Accommodation Program for short term accommodation or mid term accommodation for individuals waiting for public housing as well as public housing transfer applications;

- Subsidised mid-term accommodation sourcing;

- Domestic Violence Safety Plans and referrals to appropriate agencies;

- My Aged Care - Aged Care Assessment assistance - applications and handovers to care package case managers;

- Financial advisory;

- Mental Health and Physical Rehabilitation Hospital admission liaison and discharge planning;

- Legal concerns referrals, as well as court assistance inclusive of legal aid and linking with local advocacy agencies;

- Private Rental sourcing and assistance with concerns in current private rental properties;

- Technological hacking and fraud assistance;

- Immigration concerns and Visa applications;

- Non-government Organisations (NGO) liaison and assistive services including no interest loans, furniture brokerage and food / clothes vouchers;

- Access to government funded allied health services;

- Linking in with community mental health and drug and alcohol services;

- Child Protection (DJC) - assisting with access to removed children, family planning conferences, pregnancy planning conferences and liaison with DJC Child Protection Case Managers;

- Referrals to appropriate agencies for breakdowns in relationships;

- Companionship Card Applications;

- Taxi Subsidy Scheme Applications; Support Letters.

COACHING & CONSULTING

Service areas include: Current Case Load Reviews / File Reviews (Fortnightly/Monthly/Bi-Monthly); Business Consulting, including Development of Policies and Procedures and QMS Framework Audits.

Cherie Marantz is the Director of Outcome Health Services and principle consultant.

Since graduating as an Occupational Therapist in 1990, Cherie has predominately worked in the area of community based rehabilitation. Her areas of specialty include brain injury, spinal cord injury, dementia, musculoskeletal and psychological rehabilitation. Cherie has extensive experience in the public and private sector as both an Occupational Therapist and Case Manager. Cherie has specialised experience in driving assessments and training, and pain management ultilising the Abilita model. Cherie also has a Post Graduate Diploma in Safety Science from 2003 and has provided companies assistance with risk management and training.

Cherie’s experience includes a period of working an Occupational Therapist in a specialist based services in Canada and England. Cherie has also held a casual Occupational Therapy teaching position at Western Sydney University and has presented at various conferences. Services have been provided under privately paying clients, NSW CTP, NSW Workers Compensation, Comcare, NSW Lifetime Care and Support Authority (LTCS) and WorkCare, Workcover QLD, Life Insurance, and NDIS. Cherie is a full member of Occupational Therapy Australia and has full registration status enabling her to practice Nationally as an Occupational Therapist. Cherie is committed to maintaining a high level of professional development to keep up to date with the latest research and ensure she is able to deliver a best practice approach for her clients.
